Connected Care by Hawai‘i Pacific Health

Connected Care by Hawaiʻi Pacific Health supports Medicare patients with an at-home extension of your doctor’s office that helps you stay on top of your health between visits.

Urgent Care

Our neighborhood urgent care clinics provide high-quality care for minor injuries or illnesses when your primary care provider isn't available. You can choose to get your healthcare in person or virtually via a video visit.

Care at Kapiʻolani

At Kapiʻolani Medical Center for Women & Children, care is centered on the unique needs of women and children.

Pediatrics

Children can receive primary care from pediatricians who provide routine check-ups, preventive care, and ongoing support at every stage of growth. Pediatricians are based at Kapiʻolani, as well as at Straub Benioff clinics on Oʻahu and Kauaʻi Medical Clinic locations on Kauaʻi.

Family Medicine and Internal Medicine

For women and other adult patients, primary care is available through Straub Benioff clinics and University Family Medicine at Pali Momi Medical Center on Oʻahu and Kauaʻi Medical Clinic locations on Kauaʻi. Family medicine and internal medicine physicians at these locations provide personalized, coordinated care for adults. Family medicine physicians also provide care for children, offering support for families across every stage of life.

Care at Pali Momi

At Pali Momi Medical Center, primary care is provided through University Family Medicine at Pali Momi, where a team-based approach supports patients and families of all ages.
Family medicine physicians provide preventive care, routine check-ups, and treatment for a wide range of health needs, while also managing chronic conditions and coordinating care with specialists when needed. With a focus on whole-person care, including medical, social, and emotional well-being, patients receive personalized, continuous support throughout every stage of life.
University Family Medicine at Pali Momi is home to the University of Hawaiʻi Family Medicine Residency Program. The program is academically affiliated with Hawaiʻi Pacific Health Medical Group in partnership with the University of Hawaiʻi John A. Burns School of Medicine (JABSOM).

Care at Straub Benioff

Primary care is available through Straub Benioff Medical Center's network of clinics on Oʻahu and Lānaʻi, offering convenient access for patients of all ages.
Patients can receive primary care from pediatricians, family medicine physicians, and internal medicine physicians who provide routine checkups, preventive care, and ongoing support across every stage of life. Care is delivered through a team-based approach, with physician assistants, nurse practitioners, and other healthcare professionals working together to support patient needs and help coordinate care.

Care at Wilcox

Primary care services are provided at Kauaʻi Medical Clinic locations across Kauaʻi, offering access to care for individuals and families at every stage of life.
Patients receive care from pediatricians, family medicine physicians, and internal medicine physicians who focus on preventive care, routine health needs, and ongoing support. A collaborative, team-based approach ensures patients are also supported by physician assistants, nurse practitioners, and other healthcare professionals who help manage care and coordinate services.
