Connected Care is delivered in partnership with Cadence, supporting your Hawaiʻi Pacific Health care team.
How does Connected Care help?
- Help You Feel Better Day-to-Day. The program helps you stay healthy between visits to your doctor.
- Avoid Hospital Visits. The program helps guide you to the right care and avoid unneeded trips to the emergency department (ED).
- Support When You Need It. If you have questions about your health or don’t feel well, you can reach the care team by phone or text even when the clinic is closed.
Proactive Care Made Simple
You can use this program to get help with referrals, lab tests, and preventive care screenings that will keep you healthier. Plus, Connected Care provides diet, wellness, and exercise support that can improve your overall health. If you need help finding local resources to stay healthy, the care team can find those for you and get you set up.
Get Ongoing Support for Your Condition
Patients with high blood pressure, high blood sugar, or heart problems also get easy-to-use devices to check their numbers from home. For patients with high blood sugar, we also provide unlimited strips and lancets for your blood sugar reader. Our team reviews your numbers to make sure things look right and will call you if any additional help is needed.
How to Enroll
If you are a Hawaiʻi Pacific Health patient with Medicare coverage, ask your doctor about Connected Care. You can also call or text 808-427-9006 to learn more.
Your doctor will still guide your care in coordination with the Connected Care team.
Connected Care by Hawaiʻi Pacific Health is covered through Medicare. Most patients do not have to pay anything once their deductible is met. A member of the Connected Care team will discuss any potential cost with you before you enroll.
