Service Overview
Geriatrics, or geriatric medicine, focuses on the care of older adults. It helps people with age-related complexities, such as compounding medical conditions, medication and chronic condition management, and care planning.
Our Services
- Arthritis & Other Rheumatic Disorder Management: Assessment, diagnosis and management of arthritis and other common age-related rheumatic disorders.
- Cognitive Health Assessment and Diagnosis: Assessment, diagnosis and management of memory and mood disorders, including mild cognitive impairment, dementia, Alzheimer’s and depression.
- Falls Risk Assessment and Prevention: Evaluation and implementation of strategies to reduce the risk of falls and fall-related injuries.
- Goals of Care and Advance Care Planning: Partnership with patients and their families to explore goals of care and align these goals with their health care plans
- Medication Management: Review of medication treatments to ensure safety and avoid adverse effects.
- Referral to community organizations: For additional help and supportive services, including home health agencies, day programs, caregiver support groups and palliative care programs.
Care at Kapiʻolani
Women have unique needs as they age.
At Kapiʻolani, your Primary Care Provider (PCP) oversees your care and will connect you with the appropriate departments at Kapiʻolani and the other Hawaiʻi Pacific Health facilities. Your PCP will work with specialized providers, based on your healthcare needs, to provide high-quality, personalized care.
Care at Pali Momi
At Pali Momi, geriatric care starts with your Primary Care Provider (PCP). With a holistic understanding of your health history and your current healthcare needs, your PCP will develop a plan of care that will connect you with geriatricians, social workers, and other providers across Hawaiʻi Pacific Health facilities.
Care at Straub Benioff
Straub Benioff Medical Center – Geriatrics Clinic is a specialty consultation clinic for adults ages 75 years and older.
Our team reviews and manages the complex physical and cognitive health issues common in older adults. We also help patients reduce health risks and make care decisions that reflect their personal values.
Fellowship-trained geriatricians, a social worker and experienced clinical support staff work together alongside our patients' primary care provider to provide high-quality, personalized care.
GUIDE Model Program
Our clinic participates in the GUIDE (Guiding an Improved Dementia Experience) Model. This voluntary nationwide test is funded by the Centers for Medicare & Medicaid Services (CMS) to enhance quality of care for people living with dementia and their unpaid caregivers. The GUIDE Model aims to address key drivers of poor-quality dementia care in the following ways:
- Comprehensive assessment by a geriatrician: Includes standardized cognitive testing, quality-of-life assessment and measures of caregiver burden.
- Care navigation by a social worker: Includes caregiver education and support, referral for appropriate services, and regular follow-up.
- Availability of a 24/7 telephone advice line.
- Referral for respite services: With availability of limited funds to help pay for these services.

Kauaʻi Medical Clinic Family Practice Geriatrician Providers offer assessments, diagnosis and support plans for adults 65 years and older.
Our geriatric specialists provide care in skilled nursing and long-term care facilities. We manage patients with complex health scenarios.
